Athletic Clearance
HIGH SCHOOL
High school students who wishes to play a sport must complete the athletic clearance process each year before they can participate (this includes tryouts, practices, weight lifting and summer training programs). Oaks Christian utilizes the Magnus Health app, found on AXIS. All required information and forms/signatures are collected online. Information is saved annually in order to make the process even faster for returning athletes. Parents can use the same account for multiple students making the clearance process easier for families with more than one student-athlete. For any questions regarding Athletic Clearance, please contact the Sports Medicine team at 818.575.9241.
MIDDLE SCHOOL
Middle School students who wish to play a sport must also complete a physical and immunization form each year before they can participate. These forms are to be completed by a medical doctor, and are due to the school health office by August 1. The middle school athletic program offers nearly 30 teams for sixth through eighth grade, with some teams also open to Academy V students, as well. The middle school is part of the Delphiic League and the Tri-Valley League. Click on the tabs below to see what sports are offered and to download the physical form. In addition to league play, the middle school also offers intramural sports, which are listed on the sports flyer below.
